Molecular pathogenesis of AMD remains unclear and intricate. Although a few number of studies investigated components of oxidant/antioxidant pathways and DNA damage/repair systems separately in AMD, there is no comprehensive research study available on this issue to best of our knowledge. The aim of the current study was to investigate the evidence of systemic oxidative stress and DNA damage in patients with exudative age-related macular degeneration (AMD). This prospective controlled study comprised 22 patients with exudative AMD (67.3±2.2 years of age) and 24 age- and sex-matched (62.1±2.2 years of age) healthy controls (p>0.05). Serum total oxidant status (TOS) and total antioxidant status (TAS) were measured using a commercial kit and oxidative stress index (OSI) was calculated. DNA damage was assessed using comet assay following lymphocyte isolation from venous whole blood. Parameters including tail length and tail moment were evaluated for quantitative analysis of DNA damage. Serum 8-hydroxy-2′-deoxyguanosine (8-OHdG) levels were also measured to assess oxidative DNA damage. The AMD group had lower TAS and higher OSI values than the control group, while TOS values did not differ between the two groups (p= 0.0001, p= 0.025 and p= 0.930, respectively). Furthermore, tail length (p= 0.0001) and tail moment (p= 0.003) were significantly higher in the AMD group compared to the control group. However, serum 8-OHdG (p= 0.892) value was similar between the AMD and control groups. Systemic oxidative stress index and cumulative DNA damage appear to be increased in patients with exudative AMD.
